2022 Discover Ontario Car Rally Entry Form

This is the entry form for the Discover Ontario navigational car rally on June 25, 2022. The rally is the fourth round of the 2022 Ontario Road Rally Championship.
We are following COVID-19 restrictions for this event. In an effort to make the rally as contactless as possible we'd like you to fill out this entry form before coming to the rally. We've simplified the form so we only capture names and addresses. We are not asking for drivers license, ownership or insurance information but you must complete the self declaration stating they are valid.
The entry fee is $50.00 for the rally ($40 if both driver and navigator are TAC members). You can send an Interac transfer or PayPal to treasurer@torontoautosportclub.ca to submit your payment. Please include "DOCR" and the driver's name in the message for the payment. If you can't attend you will get a full refund.
