Walker Elementary School Parent or Guardian Report a Child's Absence Online Form
Please completely fill out this form when your child/children is or will be absent from school.

Please complete a new form for each of your children if you have multiple children that will be absent on the same day. 

This form will greatly support our record keeping and make it easier for parents or guardians to share an absences excuse with the school. 

If your child has been provided a doctor's note, you can either have the physical document brought to the teacher or email it digitally to our clerk, Kelly Hodge (hodgek@nv.ccsd.net) .  The astrix * by the title of the section means that this information is required.  Thank you!
