All Fresh Check Days must be registered with the Jordan Porco Foundation. Please complete this form to register your Fresh Check Day today!

The program fee for Fresh Check Day registration is $1000, to be paid to the Jordan Porco Foundation 45 days or more prior to your registered event date on file. 

Someone from the Jordan Porco Foundation will process your registration within 3 business days, at which point you will receive details on getting started and full access to the Fresh Check Day program toolkit.
