This form should be used to book child care at St. Mary's Primary School for children of key workers, or children who have been identified as vulnerable, as per TSSMAT's published priority lists. These can be found on the government website.
Child care at school should be used as a last resort, and only while you are at work and do not have an alternative source of child care.
We reserve the right to ask for proof of employment, and working hours.
Places will be allocated on a priority basis, we reserve the right not to offer places in some circumstances.
This provision will be under constant review.
