We are so glad you are interested in the Game on Philly program! This is the online sign-up form for the program. Please note: you must complete this online form for every child you want to register (one form for each child).  

The Game on Philly program is a completely free afterschool sports and fitness program available to ALL 6th, 7th, and 8th-grade students at our partner schools:
