I hereby authorize the staff, coaches, and volunteers to use their best judgment in any emergency.  I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence of others, the rules of play or the condition of the premises or of any equipment used.  Further, that there may be other risks not known to us or not reasonably foreseeable at this time.  I hereby release, waive, discharge and covenant not to sue ForestView Church staff, coaches, volunteers or directors from any and all liability for any injury, illness or resulting loss incurred by the participant at any ForestView activity.  I further acknowledge and approve that participants photographs may be used in promotional material for the ministry.  Additionally I authrorize my contact information to be shared with JumpStart and appropriate ForestView staff or volunteers for the purpose of communications regarding ForestView activities.
