STUDENT RULES: Please read carefully and sign this consent form to indicate your willingness to abide by the rules. Both payment and consent are required for securing your spot. As a SENIOR participating in UHS Grad Night, I agree to the following: 1. Your Ticket is NONREFUNDABLE and is NOT TRANSFERABLE 2. I understand I must present a VALID PHOTO ID upon check‐in at UHS to attend. 3. I understand bus transportation will be provided, and it is the ONLY means by which I may attend Grad Night. 4. I am fully aware that NO ALCOHOL, DRUGS, VAPE PRODUCTS, or SMOKING will be permitted. If I am in possession or under the influence of any such substance, my parent/guardian will be called to pick me up. Possession of any illegal substances also may be subject to legal ramifications. 5. I will uphold all IUSD rules of conduct. 6. I will arrive at UHS by 7:00 pm (activities begin at 5pm) to go through Grad Night check-in and board my bus. 7. I must REMAIN at Disney CA Adventure until I return via school-provided bus to UHS (estimated 3:00 am). NO EXCEPTIONS! 8. I understand that ALL items that I bring with me will be thoroughly checked by Security at UHS and by Disney CA Adventure security, which includes Security Personnel and Canines. 9.
