WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT FOR THE LCA ATHLETICS PROGRAM
Read through this waiver carefully. Athletes under 18 years of age MUST have this form completed by a parent or legal guardian. An athlete who is not a minor may complete, certify and sign this form for himself/herself. Completion of the digital signature certifies that the form has been completed by the person whose name and initials appear in the digital signature field.
A separate form must be completed and submitted for each athlete.
Understand the risks associated with participation in LCA sports and acknowledge that participation is voluntary. Agree to adhere to all sports rules and regulations set forth by LCA. If any section does not apply to you, write “N/A” in the space provided.
Seasonal Applicability: Note that this release applies to the entire sports season, from the start date to the end date, including any practices, games, or events that occur during this period.
