This form is intended for:

*Previous OTOB students wanting to register for online or in-person regular term lessons from September 2020 - June 2021. More information on regular term lessons can be found at https://ontheoffbeat.ca/fallregistration2020/

*OTOB students who have already registered for fall and would like to change times/teachers

For any new students to OTOB please visit: https://ontheoffbeat.ca/lessons-programs/private-lessons/ (this form is NOT for new students)

All forms will be processed in order of receipt. In the unlikely event we cannot find a time from your availability below we will contact you however we do recommend leaving as much availability as possible. All time slots are first come first serve.

For in-person lessons - In the event of a subsequent wave of Covid-19 all lessons will immediately revert online and the schedule will remain the same. Students must adhere to our protocols found at: https://ontheoffbeat.ca/covid-19updates/

What is your preferred lesson time (according to the teaching days listed above for your teacher). Please give us as much availability as possible (Ex. Mondays at 4:30pm is best for us but we could also do Mondays anytime between 6:00 - 7:30pm or Thursdays between 5:00 - 7:00pm.) The more availability you give us the more likely it is that we can fit you in! *

What is your preferred start date? We can register students for a start date no more than 10 days after filling out this form. *

I acknowledge that should another wave of the pandemic occur during the year OTOB lessons will resort to online lessons during the same time slots and will require four weeks' notice to cancel lessons. 30 minute lessons = $181.25 ($146.25 monthly payment plus $35 yearly registration fee)

45 minute lessons = $254.00 ($219.00 monthly payment plus $35 yearly registration fee)

60 minute lessons = $327.00 ($292 monthly payment plus $35 yearly registration fee)

Adults only - biweekly 45 minute lessons = $141.88 ($106.88 monthly payment plus $35 yearly registration fee)

Adults only - biweekly 60 minute lessons = $177.50 ($142.50 monthly payment plus $35 yearly registration fee)
