4-H Shooting Sports Annual Instructor Report
Your completed form is due December 1st.  Only one form is needed to be filled out per year. This information is to help us plan and fund the future of Wisconsin 4-H Shooting Sports. Please report annual teaching for the period of October 1 to September 30.  You will receive a copy of your responses in an email confirmation once you have completed and submitted your report.  Please forward that confirmation to your County 4-H Office, or make a copy and send it to them. Your report will automatically go to the State 4-H Shooting Sports Coordinator.  
This report may appear to be long as it covers all disciplines. The report allows you to skip through disciplines that you did not instruct. If you instruct in one or two disciplines in your county shooting sports program, completing this report will only take a few minutes. If you instruct for a camp, county fair, special interest group or Hunter Education, there are additional short sections provided in this report to complete. Be sure to include your preparation time when calculating hours for teaching.  

Areas you will be asked to report on are:
a. 4-H Events (County and club shooting sports training, 4-H project exploration, statewide invitational shoots, coaching for national shoots)
b. Special Interest (STEM, conservation groups, county/city parks classes, after-school programs, scouting organizations, NASP, etc.)  
c. Camps (Day Camps and Summer Camps)
d. Fairs (State and county)
e. Other Events (NWTF Jakes, DNR Hunter Education, Mentor Hunt, etc.)

For State Training Team members (Level 2) and National Instructors (Level 3), you will have a section at the end to report the number of adults you have taught.
