Please use this form to request a regular booking for your child(ren) at breakfast club. There are separate sections of the form to book up to 4 children. New bookings must be made by 12pm Thursday each week to start the following week and are allocated on a first-come, first-served basis. If your child has already been to breakfast club this term you do not need to complete a form every week.
Emergency one-off bookings for breakfast club must be made by telephoning the school office and will only be accepted if there is space available. Please do not turn up at breakfast club without a booking.
Breakfast club costs £5 per day per child with breakfast included. Please pay for Breakfast Club in advance using the following account information:
