I/We agree to adopt a dog from Paws Restart and will abide by the terms of the agreement.

I/We agree to abide by Paws Restarts' recommendations regarding the care and welfare of my adopted dog.

I/We or any member of my/our family have never been subject to a formal complaint or legal action for cruelty or neglect of any animals and have never owned animals which have been confiscated by any agency for violations of any laws, regulations or animal adoption agreements.

I/We acknowledge that Paws Restart is not responsible for my/our adopted dog after the date of approval of my/our application. Once I/We sign agreement to all  terms and have taken possession of my/our new adopted dog, I/We will from that day onwards accept full care and responsibility for the dog that I/we have adopted.

I/We agree to indemnify Paws Restart for any liability, costs or expenses that may arise as a result of my actions or omissions (including negligence) or of my/our adopted pets condition, health or behavior.

I/We acknowledge that Paws Restart is NOT obligated to take the dog back after the adoption is finalized. However, if the dog needs to be rehomed in extreme circumstances, I/we will contact the Paws Restart in the first instance to discuss my/our situation. 

Only when so required I/We give consent to Paws Restart to take urgent action without notice if they determine that the life of my adopted dog is at risk.

I/We agree to the adoption non refundable fee of $550 paid before dog is adopted into our care.  This fee covers microchipping which is registered on the NZCAR, vaccinations including Vanguard Plus5, Leptoguard and Vanguard CC B Oral, up to date flea & worm treatment, desexing at a Vet's North clinic if not already done, a weeks supply of their current food, lead, collar, their blanket and favourite toy.  Council registration is the responsibility of new owner. Adoption fee goes into Paws restart animal rescue charity account.  03-1322-0355327-000
