Once again, the Brookview PTO has created a form to facilitate the creation of individual classroom directories! This will be the only means by which Classroom Coordinators will communicate information for classroom parties (Fall, Harvest, Give Love) as well as Staff Appreciation events throughout the year. It can also be used to contact other families for birthdays, playdates, and other activities for your child(ren).

Teachers do not share contact information for students, so this will be the only way to share classmate contact information.

Once collected, this information will be shared ONLY with the parents/guardians in your child's classroom who chose to participate in this Classroom Directory. That means if you choose not to sign up for the Classroom Directory, you will not receive contact information for the other families in your child’s class.

The PTO may also use the information to occasionally send out information that is of interest to the entire student body. BCC email format will always be used.

Please submit ONE form PER CHILD, not per family.  You will receive a Response Receipt with a copy of your submission.  If you need to make updates/changes, please email brookviewpto@gmail.com.  Do not submit more than one form per child.
