In order to support the NHS Test and Trace programme, we need your consent under GDPR legislation to share your details with NHS Test and Trace, if asked, in the event that this information is needed to help stop the spread of coronavirus. The giving of consent is not compulsory and if you give it, you may withdraw it at any time. However, once you have given it, we will hold it for as long as the Test and Trace scheme exists or until you withdraw your consent. For parents and guardians, please give the names of children under 18 covered by this consent. Once the form has been received, we will deem you to have given consent and will keep a record of each time you attend services at church and hold these for a period of 21 days before destroying them.
