Lamar Blended or Virtual Learning Enrollment Form
Complete this form to specify which learning opportunity you want your student enrolled in for the first 9 weeks of the 2020-2021 school year.

You will need to complete a form for each student/child you have enrolled in our school.

Students enrolled in Blended Learning must stay for the entire 9 weeks period. Students enrolled in Virtual Learning can move back to Blended Learning at any time, however, they will be required to remain in Blended Learning until the end of that 9 weeks period.

Blended Learning – Combined use of traditional face-to-face instruction integrated with Virtual Learning due to when a student(s) is being sent home due to COVID-19, Influenza, or other communicable diseases.

Virtual Learning – When a student attends off campus only to view video lessons, complete assignments, and turn in assignments through an internet connection. (Students must have access to the internet to download videos and assignments and to upload their classwork.)

All students will be provided a Chromebook at the beginning of the school year. The student/parent will be accepting responsibility for the care of the device until they are returned. This enrollment document will be printed and the Chromebook number assigned to your child/student will be added the document for your signature. New Attendance Procedures will be made available ASAP. It is important for everyone to understand that attendance for students enrolled in Virtual Learning will be linked to the student's completion of classwork. (If a student does not turn in the work, the student will be counted absent!) (If a student does not turn in doctor notes or parent notes, the student will receive an unexcused absence. Please read policy 4.7 and 4.8 on student attendance and make-up work.
