**IMPORTANT INFORMATION - PLEASE READ ALL INFORMATION BEFORE COMPLETING REGISTRATION FORM**

PLEASE NOTE THAT DUE TO THE CIRCUMSTANCES SURROUNDING COVID-19, CAMP COSMOS WILL BE RUNNING AS PLANNED UNLESS IT IS DEEMED UNSAFE TO DO SO. ANY AND ALL CHANGES THAT COME TO OUR CAMP PROGRAMMING DUE TO COVID-19 WILL BE SENT OUT IN A NEWSLETTER AND POSTED ON OUR PLATFORMS.

 Please note that ALL children must wear masks in indoor common areas, and children 7 and up must wear masks inside at all times. Both cloth AND disposable masks are acceptable at camp and disposable masks will be available on site. Children must observe SOCIAL DISTANCING (2 meters) while indoors and maintain 1 meter of space between each other while outdoors. Children are not permitted to switch camp groups. Camp Cosmos will provide alcohol hand sanitizer and disinfectants but encourage parents to pack extra in their child's backpack.

Within 24 hours of registering, you will receive a PAYPAL REQUEST FOR THE DEPOSIT. The amount of the deposit is equivalent to DOUBLE YOUR WEEKLY CAMP FEES and will be held until the final two weeks of camp, PLEASE NOTE THAT YOUR REGISTRATION IS NOT COMPLETE UNTIL YOUR DEPOSIT HAS BEEN RECEIVED. Spaces at Camp Cosmos are allotted on a first-come, first-served basis.  Once the deposit is paid, you will receive an email of confirmation. If you do not receive a confirmation email within 24 of registering, please get in touch with us.

This form is to register for our DOWNTOWN camp, located at Saint James United Church, 1435 City Councillors St, Montreal, Quebec H3A 2E4. PLEASE NOTE: THIS LOCATION IS LOCATED UP A FLIGHT OF STAIRS.

This form will take 15-20 minutes to complete. All information in this application will be kept confidential within the Camp Cosmos team.

For each child you wish to register you must complete a separate form. Campers must be at least 5 years old and have completed kindergarten, and the maximum age is 11 years old.
