CAPA IT Club Signup Form
We are CAPA-MC IT Club, and our mission is to provide free, high quality, and easily accessible IT services to local clubs, organizations, and businesses, particularly surrounding website creation and design. We design and create websites collaboratively on programs such as Wix.com and WordPress.

Requirements for new members:
1) Must be a middle or high school student in MCPS
2) Commit to IT Club, attending at least 60% of meetings and events
3) Interested in STEM fields
4) Finish all assigned activities on time

To sign up to volunteer with CAPA-MC IT Club, please complete the following 3 steps by March 31, 2024. Any missing steps may result in the loss of volunteer opportunities.
1. Fill out the registration form on this page.
2. Have a parent/guardian manually sign the WAIVER OF LIABILITY STATEMENT below, then scan and email it to CAPAITClub@gmail.com
3. Have both the volunteer and parent/guardian manually sign the VOLUNTEER CODE OF CONDUCT, then scan and email it to CAPAITClub@gmail.com

**Please note that IT Club is an organization under CAPA-MC (Chinese American Parent Association of Montgomery County). CAPA requires the parents of the volunteer(s) to be members of CAPA. It will be required to pay the membership fee ($30 every two years) to CAPA at its website (https://www.capamc.org/).
