SMS Job Shadowing Application
"Job Shadowing Day" is on Friday, February 2, 2024. This special day is when students and parents work together to schedule a shadowing experience for the student.  The student will have to be transported to the location and will be at the location all day instead of coming to school. We encourage the student to visit a career that we discussed at the IGP meeting. This is a career the student would like to pursue when he/she is an adult. 

To be excused for the entire day, the students MUST complete these 3 things:

1) Complete the parent permission form(ask your teacher for one or pick one up in the counseling office), and have your parent sign it. Return the permission form to the counseling office basket labeled, "Job Shadowing" asap or before Wednesday, January 31st.

2)Complete the following survey and submit it by Wednesday, January 31st. You must know where you are going before you complete the survey.  Please make sure you have confirmed with your shadow mentor/business before you submit this form.

3) Complete the Job Shadowing Observation Report during the Job Shadowing experience.
