I, the legal parent/guardian of ______________________________________, authorize and consent to the use of images and videos take of my child listed above, with or without their name(s), by Anacortes Lutheran Church for the purposes including but not limited to: promotional materials, internet posts on the church website, church social media, and other social media sources.  

I do this with full knowledge and consent to waive all claims for compensation for use or for damages. I release Anacortes Lutheran Church, its officers, trustees, employees, and agents from liability for any claim by me or any third party in connection with the use of the image of my child.
