2023 Community Village Eligibility Form (Group)
This form is due by  Monday, April 17, 2023. The dates for the 2023 Oregon Country Fair are July 7, 8 and 9. Please fill this form out completely. To participate in Community Village as a group, the group must be determined to be eligible. Please review the guidelines at https://communityvillageocf.org/community-village-guidelines/ .  If you are unable to submit a form electronically, please ask you Booth Coordinator to help you or connect you with someone in your booth who can help you. You are responsible for ensuring that your form is submitted by the deadline.

Eligible groups will be granted a limited number of passes. The rule is 2 worker passes OR 1 worker and 3 worker day passes, or 6 Worker Day Passes. S.O. passes are also limited.
