Litter nomination must be done no later than 56 days (8 weeks) after whelp to prevent late fees.
Payment must accompany this form and can be sent via PayPal to
daca@dogousa.org.
Please email receipt of payment to Futurity Director at
show.committee@dogousa.org as proof of payment.
NOTE: At least one breeder must be a member in DACA for at least 6 months prior to nomination - non member fee applies if one listed breeder is not a member of DACA.
