Directions: After reading the Rock County High School Application for Account and Terms and Conditions (found on the school website), please read and fill out the appropriate portions of the following contract completely and legibly.  The signature of a parent or guardian is also required.

CONTRACT PORTION OF DOCUMENT
I have read the Rock County High School Terms and Conditions.  I understand and will abide by the stated Terms and Conditions for Rock County High School.  I further understand that violation of the regulations is unethical and may constitute a criminal offense.  Should I commit any violation my access privileges may be revoked, school disciplinary action may be taken and/or appropriate legal action.

PENALTIES
Additionally, to the extent that it can be reasonably determined by the administration what fees if any have been incurred by a person for non-authorized purposes, it shall be the policy of this district or seek reimbursement and full restitution from the student or his parent or guardian, for use of the Internet in a manner inconsistent with this policy.
