2023 Continuing Formation Form

Please use the following to report what you have done for ongoing formation during the year of 2023. All deacons who are active status are required to have 20 hours of ongoing formation each year. These 20 hours can be accomplished in many different ways, so please check all that apply to you.

As a reminder, all deacons are expected to make an annual retreat as well and strongly recommended to have a spiritual director with whom he meets on a regular basis.
