Please fill out the form below for each player that will be participating in the Arizona Storm FC Girls Spring Clinic. If your daughter is currently registered for another ASA affiliated club, they must bring a signed loan form. 

Who: Girls born between 2016 (U7) to 2007 (U16)
When: Saturday, April 1st, 2023
Time: 8:30am to 10:00am
Location: Trivium Prep High School - 2001 N. Bullard Ave, Goodyear 85395
Cost: Free
Open to all girls players from all clubs

Players will need to bring the following items to the clinic.
  • Plenty of water
  • Soccer ball
  • Appropriate footwear and attire

Recognizing the possibility of injury associated with the sport of soccer I hereby grant permission for the above mentioned player to receive medical treatment if needed. Therefore, I will assume the financial responsibility for any medical treatment my child receives. Additionally, I will not make any claims against the Arizona Storm FC and/or its staff in the event of an injury. My child is in excellent physical condition and is allowed to participate in a strenuous activity such as soccer.
