COVID Screening: Sandowne YDP
Parents: All parents of children enrolled in the Youth Development Program (YDP) for before school at Sandowne  PS are asked to complete this form each day your child is scheduled to attend. The form must be completed prior to arrival at the school. Upon arrival, staff will conform its completion and take your child's temperature to complete their screening.  

Staff: All employees within the Sandowne  youth development program (YDP) - including those working before and/or after school, are required to complete this screening questionnaire each day prior to reporting for work. A temperature check is required upon arrival at the school. If you have any symptoms or feel ill, please do not report to work. Instead notify your supervisor immediately.

SCREENING RESULTS
If you answered YES to any of the questions above, and/or you (i.e., staff) or your child has a fever above 37.8° C, he or she will not pass the screening and is not permitted to attend the before school YDP program.

If your child is ill, please keep them home. If they have any of the COVID symptoms, you are advised to consider getting your child tested for COVID. See https://www.regionofwaterloo.ca/en/health-and-wellness/community-assessment-centres.aspx for testing centres. Alternatively, you may consult your doctor and seek a doctor's note confirming that their symptoms are not COVID-related and they are okay to return or you may have your child and family self-isolate for 14 days before returning. Please note that we are required to notify Public Health of any COVID-related symptoms and must file a report with the Ministry of Education for any child displaying 1 COVID symptom who has been referred for testing.
