**Virtual Learning changing to Face to Face: If a student has been attending virtually and wants to change to face to face, submit this form. This process takes anywhere from 3-5 days to complete.

**Face to Face Learning changing to Virtual Learning: Students that have been attending school face to face can change to virtual learning at any time. This form must be submitted for the child to start virtual learning the following day.
