I understand that upon enrollment, I will be charged a non-refundable $35 enrollment fee, per class that I am enrolling my child in. *

I understand that if I do not choose to pay my child's tuition in full upon enrollment, monthly tuition will be automatically processed on the 1st of each month, September through May. If the 1st of the month falls on a bank holiday or a weekend, tuition will be charged on the Friday prior to the 1st. I agree to allow Elite Performing Arts Conservatory to charge my card on file for agreed upon tuition and recital ticket/costume charges.

I understand the following tuition fees:

$50 per month for 30 minute weekly classes, $70 per month for 45 minute weekly classes, $125 per month for Company Team Rehearsal Classes, $300 per month for Weekly Private Technique Sessions. If my child is enrolled in 6 or more weekly classes, I will meet the criteria for the $400 per month unlimited class tuition.  

I understand that, if my card on file is declined, I will be charged a $35 per week late fee.

I understand that there are additional fees associated with the Winter Recital and the Spring Recital (notifications of recital costume/ticket fees will be sent out via our studio Remind page). 

I understand that Company Team dancers will also incur competition entry fees and costume fees. 

I understand that upon enrollment, my student's tuition is prorated into 9 monthly payments, which will be processed regardless of absences or holidays. I understand that I am enrolling my child for the full season of dance (September through May) and I will be charged monthly tuition through May 2025 regardless of absences or withdrawals. In the event that my child decides to withdraw prior to the end of season, I will be charged a $50 withdrawal fee for each class that my child withdraws from and I will be responsible for tuition payments through May 2025.

I understand that all payments are non-refundable.

I also understand that with sports and physical activity, there is a risk of injury. I agree to not hold Elite Performing Arts Conservatory, its Owner, Crystal Frank, and its staff responsible for any injury sustained at the studio or at studio events.
