This serves as the proxy vote authorization form for the Annual General Meeting (AGM) of the year 2021 of the JC School of Public Health and Primary Care, Undergraduate Alumni Association, The Chinese University of Hong Kong. The captioned AGM will be held at 7:30pm on 5th November 2021, in Tutorial Room 1 & 2, 1/F, School of Public Health and Primary Care, Prince of Wales Hospital, Shatin. Registered members of the Association could choose to participate in our AGM in person or via Zoom. Deadline for submission of this form is the 11:59pm of 4th November 2021. Please note that registree has to be registered member of the Alumni Association. All your personal details collected in this e-form will be destroyed 3 days after the Annual General Meeting.
