Appointment and Payment Information:

Instructions for Completion:
Answer each question to the best of your ability. Remember, there are no "right" or "wrong" answers. Providing detailed information allows us to tailor our consultation or program to your specific needs.
If your dog has displayed concerning behaviors, such as growling or biting, please indicate by checking the relevant box. This will prompt additional questions specific to your situation.
This form should be completed by the pet owner or an adult legal guardian.
After Submission:
You will receive an email confirmation immediately after submission. Within 1 business day, we will follow up with an email to schedule a Discovery Call, if you haven't already scheduled one.
If you have multiple dogs needing assistance, please complete a separate form for each one.
For behaviors involving aggression, or if you're uncertain about your dog's reactions, please do not attempt to replicate any scenarios from the questionnaire.
Please submit this form at least 72 hours before your initial appointment.
Consultation Guidelines:
All adults regularly interacting with the dog should be present during the consultation for the most comprehensive advice.
Given the limited number of weekly appointments and the time allocated for each, please inform us promptly if you need to cancel or reschedule.

Services are provided virtually via Zoom and/or pre-recorded video. Ensure you have a stable internet connection and a compatible device (smartphone, tablet, or computer).
