6th graders at O’Hara Park School are required to choose one elective course. Please fill out your personal information and make sure you fill out this form in its entirety. Failure to do so will make it so you do not get your top choice. We will try to accommodate everyone's choices, but since the schedule is a complex thing, we can't always promise it.
