This form only needs to be completed if you intend to change the instructional method of your child(ren) starting January 4th.

Face-to-Face - Choosing this option means your child will attend school Monday - Friday.

Virtual - Choosing this option means that your child will work online Monday - Friday during regular school hours and have access to internet throughout the entire school day.

*All changes must take place between January 4 and January 15th. After January 15th, no changes can happen until March 22nd.
