Faith formation registration form for children 1st - 6th grade.

Our Whole Community Catechesis family faith formation program will be offered this fall. After a survey of St. Madeleine Sophie families, we have chosen a hybrid approach to our faith formation year. We will meet in-person once a month, virtually once a month and have home study. Our in-person session is structured to have the student attend with a mentor - generally a parent or other family member. The in-person sessions will be on Wednesday evenings from 6:30-7:30 pm. See calendar for dates. It is our preference that participants attend  in person sessions to help build community, but if you are uncomfortable being in person, you can attend the virtual sessions and home study the other sessions. Please indicate this wish in the comment box below.

Registration fees:  $70/1 child, $90/2 children, $110/3 children
SMS School families: fee applies to Sacramental Preparation classes only

While faith formation is online, our catechists will host live Zoom meetings with each grade. Zoom meetings will be recorded. I acknowledge that my child will participate in these Zoom meetings and that a parent or another adult specified by the parent will be available to participate as well. *
During our online faith formation experience, St. Madeleine Sophie will use the platform Flipgrid to help increase student engagement. Flipgrid allows students/families share short video responses based on questions posed by the catechist. The settings on these videos are set to private so they can only be viewed by the catechist and Director of Children's Ministry. Only after being reviewed and allowed by the catechist or Director, can they be seen by the rest of the class.
