Thank you for volunteering!!! This form is for members who volunteered during the 2024 Rep Evaluations. Members are required to track their hours after EACH shift in order to receive credit. Please ensure that the date entered on this form corresponds with the date of your scheduled volunteer shift. We thank you in advance for assisting us in tracking your hours as quickly and accurately as possible. Please read each question carefully. Upon completing this form, you will receive a confirmation email with a copy of your submission. If you have made an error or missed something, you will have the option to edit your submission.
Once a member has completed four (4) hours of Association level volunteering, LMHA will review and determine whether their Volunteer Bond has been fulfilled. Should this be the case, LMHA will destroy the members post-dated $150.00 Volunteer Bond deposit cheque and / or mark the member as having fulfilled the Volunteer Bond requirement for the 2023-24 Season.
Please ensure you include ONLY the roles and hours you volunteered during Rep Evaluations on this form and not volunteer hours from another event.
