Many significant decisions must be made for reopening Worth 127, and your input is important. Please answer the following questions regarding the start of the 2020-21 school year.

As you respond to the questions below, please note that as of June 25, Illinois is in Phase 4 of the Restore Illinois Plan, and Worth 127 students are able to return to school for in-person learning.

In Phase 4, the Illinois State Board of Education (ISBE) and the Department of Public Health (IDPH) require that:
a. Masks or face shields will be required to be worn by all faculty, staff, and students.
b. Social distancing protocols must be followed as much as possible.
c. Extensive cleaning will occur throughout the day and in the evening.
d. Large group gatherings or activities over 50 people are not allowed to meet.

Please note that these requirements may change by the time school resumes, and Worth 127 will continue to update the Return to School plan as ISBE/IDPH guidance evolves.

The information gathered from this survey will be used to guide the reopening of schools.

Attending school is not safe, even with IDPH guidelines in place.

My student(s) is/are at a higher risk for contracting COVID-19 due to lung disease, asthma, heart condition, immunocompromised, obesity, kidney disease, liver disease, diabetes, or other medical condition.

I have an immediate family member that is at higher risk of contracting COVID-19.

My student(s) have anxiety regarding returning to school.

I will wait until there is a proven vaccine for COVID-19 before sending my student back to school.
