Student/Parent Book Club Interest Survey
Hello!  I'm Mrs. McClure the Librarian at Cheek MS, and the sponsor for the student/parent after school book club.  Please, BOTH student and parent, complete the following questions using two separate form submissions so I can gauge interest and availability.

Each family is responsible for obtaining a copy of the chosen book club books, either by purchasing a copy or visiting a public library.  Please don't let this keep you from participating - I may be able to find some cheaper, used options available for you.  :)   The club should be able to read 4 - 5 different books.    *
