Please answer the following questions honestly and to the best of your ability if you believe your child:  is infected/has been diagnosed with COVID-19, has been in contact with someone who is sick with COVID-19 symptoms or has tested positive for COVID-19, or has been in close contact with an individual who has been requested to self isolate.

If you have questions regarding this form, please call and leave a message for your School Nurse Anali Velez at 575-637-3234.  She will return your phone call within 24-48 hours.

If your child is not feeling well, has tested positive for COVID-19, or has been in direct contact with someone who has tested positive, please keep them home and wait to hear back from your child's school regarding when your child is able to return to school.
