Please fill out the following form and at the end there is a spot to submit your video. It will take about 3 minutes and I’ll be in touch when I’m ready to use your video! This form can be edited -- so if you submit a video now and I haven’t used it within a few weeks, you are welcome to change your video at a later date. Submit as often as you’d like.
