Please complete this form in its entirety. This IT Service Request Form is only for G. James Gholson Middle School students that are experiencing issues with their Chromebook. Once the service request is completed, GJGMS' IT will be in contact with you via phone, email or in person(if during school hours) to examine the issue to determine if it needs to be repaired or replaced. Please note that if a device needs to be repaired or replaced, there will be a fee associated (1st incident $30, 2nd incident $200) that must be paid before the device is repaired or replaced. Fees must be paid by money order or MySchoolBucks only.
