2021-2022 Financial Aid Request Form
We firmly believe that financial strain should never cause a student to feel that they cannot fully participate in the Fairfield Choir Department at the musical level that best suits them. We will do our best to assist anyone with legitimate need.

In order to receive financial assistance or an extended payment plan from the choir department, we ask each student and their parent/guardian(s) agree to the following stipulations. We appreciate your cooperation with this matter, and we will continue to do everything in our power to assist you with the financial commitments of this choral program.

1. Complete this Financial Aid/Payment Plan Application including a brief description of your financial situation and a monthly payment that you can afford on a regular basis.

2. Students will participate in all choir fundraisers as this is one of the avenues used to raise money to provide scholarship money to those in need. However, the money you personally raise does not automatically pay off you account - it goes into our general fundraising account.

3. Both students and parents will be available to give their time and talent to the program as often as possible! This includes helping with various projects, such as preparing for concerts, cleaning the room, organizing costumes, assisting on parent and volunteer committees, etc. when needed.

Note: The information provided in this form is confidential and will only be discussed with staff and the group treasurer's should you be awarded any financial aid.
