As you will have seen our winter party and
awards night will take place from 18:00-21:30 on Monday the 18th of
December at the Mildmay Club. As well as a range of awards we will have a
DJ and a photobooth for members to capture memories of the night which should
be a lot of fun. This form is to request your tickets and, if you are willing
to, share dishes/food you would be willing to bring along.
The tickets are priced as follows:
- Children under 18* - £7.50
- Adults - £10.50
- Group/Family of 3 - £19
- Group/Family of 4 - £24
- Group/Family of 5 - £28
* All under 18s must be accompanied by an adult.
Once you have completed this form you will be sent
an invoice (from our payment provider "Xero"). As soon as you
have paid that invoice your ticket(s) will be secured and your name will be
added to the list for the door. Space is limited at the venue so please do

For those kindly bringing food we would like to send you a label
so that you can identify ingredients and allergens when you leave the dish on the table. We might also contact you to ask if you could bring alternatives in the event that we end up with lots of similar ideas.
The information you provide will be treated in
strict confidence as per our privacy statement. Information about the party
will go our Social Secretaries, Treasurers and our Executive Committee for the purposes of running the event (contact
details at end of form).
