This Google Form is replacing the 6 page paper forms of past years and must be completed for each child you are looking to enroll in MRASP. it is due by June 17th. While the final page will feature a check box to serve as confirmation of an above health clause, we will also be collecting ink signatures on that final page at pickup during the weeks of 6/7 and 6/14.
