All volunteers under 18 must have this form completed by a parent or legal guardian.  Youth volunteers will not be accepted or scheduled to volunteer without it. This is not the application to volunteer, but a parental consent for those minors who wish to volunteer.

I, the parent/legal guardian of the Child Volunteer described above, do for myself, for the other parent of the child, and for and on behalf of my child volunteer hereby release, forever discharge and agree to indemnify and hold harmless White Stone Project / The Big Ticket Festival, and its officers, directors, employees, agents, contractors and subcontractors, from and against any and all liability, claims, actions or causes of action, suits or demands for personal injury, bodily injury, sickness or death as well as property damage and expenses, that may arise from the child volunteer’s activities for The Big Ticket Festival as stated above.  I also accept the Agreement below that my child has signed or committed to online.I hereby authorize a Big Ticket Festival staff person, as agent for me, to consent to any X-Ray examination; medical, dental, or surgical diagnosis; treatment; and hospital care advised and supervised by a physician, surgeon or dentist (as appropriate) licensed to practice under the laws of the state where the services are rendered, either at a doctor’s office or in any hospital.  I expect to be contacted as soon as any emergency arises involving my child.

I understand that The Big Ticket Festival reserves the right to charge me for the gate-ticket price for any of the following reasons: 1) Not attending all my shifts without prior notice.  2) Not completing all of my scheduled shifts.  I agree to serve with a Christ-like attitude and a servant’s heart.  I hereby release The Big Ticket Festival, its staff and sponsors from responsibility for any injury or illness that I may sustain while volunteering at Big Ticket.  I do for myself hereby release, forever discharge and agree to indemnify and hold harmless The Big Ticket Festival and its officers, directors, employees, agents, contractors and subcontractors, from and against any and all liability, claims, actions or causes of action, suits or demands for personal injury, bodily injury, sickness or death as well as property damage and expenses, that may arise from my volunteer activities for The Big Ticket Festival as stated above.
