EXHIBITION DATE: August 10, 2019

By filling out the below form, I (the Artist), hereby release my artwork for sale and loan at MINT (the Gallery) for the duration of the exhibition as stated above. The Artist is responsible for all transportation of artwork to and from location. The Artist will need to install their work upon arrival during the appointed drop-off period.

MINT will pay the Artist any commissions from works sold via PayPal within 7 days of the exhibition's closing. The Artist agrees to pick up any unsold artwork from the Gallery prior to August 31, 2019. If the Artist is unable to do so, then the Artist agrees to contact the Gallery to make special accommodations for unsold artwork and to coordinate return of the unsold artwork in that timeframe. The Artist understands that if unsold artwork has not been picked up prior to August 31, 2019 and the Artist has not contacted the Gallery to make accommodations for unsold artwork, this unsold artwork will become property of MINT.

By completing the form below, the Artist acknowledges having read and agreed to all term and conditions stated above and to release all artworks listed on the second page of this document to MINT Gallery for the stated duration of exhibition.
