Fill out and submit this form to create a SmartWaiver waiver link for your Section or Chapter. The Golden office will name the section/chapter as the event manager by default (First name: Oregon / Last name: Section).

Waivers are good for the calendar year they are signed. Sections and Chapters will maintain their own master list of participants who have a waiver on file, and can access the list through SmartWaiver.

You will be unable to access participant personal information (address, phone number, emergency contact info) from the waiver, so you will still need to have them fill out a separate registration.
