If completing it separately, I ask that both partners complete one form each. If completing together, I ask that both partners contribute to the answers.

Response Descriptions 
If you do select an item as a "coaching topic", that means you have interest in a guided discussion with your partner and coach. Select as many as you want to see where your curiosity and desires are. We can sort out the number of coaching sessions later. 

When looking at a topic, especially if you are interested in a coaching discussion, consider if you have "similar views" or "different views." Also consider if this is an area of current or ongoing "conflict."

If you are not interested in a topic, simply click "no interest."

Additional information - You can, but are not required to, add information. 

The additional information may include specific questions, aspects of the topic you are wrestling with, more discrete areas of interest, a story to share, or a curiosity about this area. 

There may be areas that you realize are important but simply don't know enough about, and that can be a great reason to pursue coaching.
