One-Time Sign Up Sheet
PLEASE READ BEFORE SIGNING UP:
-For the first session, please schedule at least 48 hours in advance so that our tutors can prepare
-After the first session, if you would like to set up more sessions with your tutor on a consistent basis, please reach out to them 24 hours in advance
-Once you fill out this form, within 1-2 days you will receive an email from a tutor. Please check your inbox and spam folders for this email and confirm the session. If you do not confirm the session, we cannot guarantee the session.
-If you need to cancel a session, please let your tutor know ahead of time so that they do not wait for you in a Zoom/Google Meet meeting
-It is NOT the tutor's role to do the questions for the student. Please make sure you attempt all material before the tutoring session.

DISCLAIMER: Students should not agree to meet tutors in person and communication should be limited to video calls and emails. By filling out this form, you understand that any action taken beyond this disclaimer is at your own risk. PeerLab is not liable for anything that occurs outside tutoring sessions. The student is responsible for reporting any actions of misconduct in the tutoring session.
