Thank you for being a special contributor to the 2022-2023 Yearbook. Your customized message can be written by a parent, guardian or relative, in dedication to any currently enrolled HNMCS student. The design includes unique text and photo(s) to be featured in a special section of the yearbook. This is a great way to celebrate your student's accomplishments during the year, or to mark their achievements or academic milestone.

Please note: No preview designs or proofs will be provided. Accordingly, please ensure the Dedication Text added to this form is thoroughly proofread before submitting and does not exceed the maximum text lengths listed for each design size. This form must be filled in and submitted by Wednesday, May 31, 2023.

Photos can be sent directly to yearbook@hnmcs.ca. Photos must be sent in by no later than Wednesday, June 7, 2023. Your support of HNMCS and our official Yearbook is greatly appreciated!
