Request form for Fabric of the Month.

This is your chance to get a surprise in the mailbox each month. You'll need to choose the count and type of fabric you'd like, and I'll surprise you with a custom created colorway for that month. It might be any color or mix of colors depending on my inspiration that month.

You'll be invoiced at the end of one month for the next month's FOTM. I will then ship fabrics in the first two weeks of the next calendar month providing the payment is received on time. You are free to cancel your subscription at any time.
