This is a booking form. 
Once submitted, your responses will be processed as soon as possible and an invoice will be sent to your email. This invoice must be paid by the due date and is confirmation of your program and class times. All alterations/transfers/no shows and withdrawals will incur a $33.50 administration fee.

Dive Stars -  (50 minutes)                                   8:50-9:40am,         9:40-10:30am
Pre-requisite: purple swim level+  

Artistic Stars -  (50 minutes)                            8:50-9:40am,         9:40-10:30am
Pre-requisite: pink swim level+         

Swim/Dive/Synchro Combination Bookings
30% discount applies when booking a dive program or a synchro with your swim lesson.  

*We will co-ordinate swim/dive/synchro times, if you book multiple programs
Fees: Swim/Dive/Synchro
Swim Lessons: $134.00 per week
Private Lessons: $268.00 per week

Dive Stars Lessons: $134.00 per week
Artistic Stars: $134.00 per week (FREE for anyone new to Artistic Swimming)
 
Swim + Dive Stars: $134.00 + $93.80 (30% discount)
Swim + Artistic Stars: $134.00 + $93.80 (30% discount)

Booking Form Procedures.  Once submitted:
* your booking time request will be considered and processed then
* an invoice will be sent to your email confirming class time
* we will contact you with alternative times if your preference is unavailable
* invoices must be paid within 5 days. Students will not be able to participate until payment has been received.
* after 5 days, unpaid Invoices/bookings/cancelations will incur a $33.50 administration fee and bookings will be voided
* credit will be applied to your customer account upon receiving the student's medical certificate for missed classes
