We would like to welcome you to our ASCS Sports Program. Being part of a sports team teaches our children so much more than just playing a game.It teaches them “team work” and responsibility. We look forward to a great year!

This form must be on record in order for your child(ren) to attend an away game and/or practice. It will allow us to no longer require a“note” for each event, as we will have this form on file. HOWEVER, YOU MUST make the arrangements for each event with the parent(s) directly.

An EMAIL to the coach that morning to let them know who the child(ren) will be riding with and if a sibling is to be picked up from carline, will be required. If your athlete is required to “pick up” a sibling on their way to the cafeteria, (even if you are picking them up) please make sure they are all aware of the arrangements so they can responsibly make the pick up and get to the location on time.
